Not necessarily. Most people with shoulder pain do not need an MRI as the first step. A clinical assessment can often identify the likely cause and guide initial treatment. An MRI may be recommended after significant trauma, when a major rotator cuff tear or other structural problem is suspected, when symptoms persist despite appropriate treatment, or when imaging is likely to change the treatment plan.
If you’re wondering whether you should get an MRI before starting physiotherapy, the answer depends on how your shoulder pain started, your symptoms, examination findings and how your shoulder is functioning. The important question is not simply whether an MRI can show a problem, but whether the scan will change what you need to do next.
Do All Shoulder Pain Patients Need an MRI?
No. Most current guidance does not recommend advanced imaging as the first investigation for ordinary, non-traumatic shoulder pain.
The Evidence-Based Interventions guidance from the UK Academy of Medical Royal Colleges recommends that a plain X-ray, not an MRI, is the first line of radiological investigation for most routine shoulder problems, with ultrasound, MRI and CT reserved for cases where the diagnosis remains unclear after clinical assessment, or where a specialist is planning definitive treatment.
There is a second reason clinicians are careful here, and it is the one most patients have never been told: a scan of an adult shoulder almost always finds something.
- In the FIMAGE study, published in JAMA Internal Medicine in February 2026, MRI scans of 602 Finnish adults aged 41 to 76 found rotator cuff abnormalities in 98.7% of participants. These abnormalities appeared in 96% of shoulders that had no symptoms at all, and the concordance between what the scan showed and what the person actually felt was poor.
- A population screening study in Japan found full-thickness rotator cuff tears in 22.1% of residents examined. Around two-thirds of those tears caused no symptoms. Prevalence rose steadily with age, from about 11% in people in their fifties to nearly 37% in their eighties.
- A 2025 systematic review in JOSPT pooling 53 imaging studies reached a similar conclusion: rotator cuff findings on imaging are common in shoulders that do not hurt.
This does not mean scans are useless. It means a finding on a scan is not automatically the cause of your pain. If you are over 40 years , there is a good chance an MRI will report tendon changes or a partial tear that has been there for years and has nothing to do with why your shoulder hurts today. Acting on that finding alone can lead to treatment you did not need.
The practical position is this: you often do not need a scan to know what to do next. Start with a proper clinical assessment, and use imaging when it is likely to change management.
When Do I Need an MRI for Shoulder Pain?
In several situations, imaging genuinely changes the plan and should not be delayed.
After a significant fall, accident or dislocation. Trauma raises the likelihood of a fracture, a large tear, or damage that a physical examination alone cannot confirm. An X-ray usually comes first to rule out a fracture; MRI may follow to assess soft tissues, such as muscles, tendons, ligaments, the labrum, bursae, joint capsule and fascia.
When a complete rotator cuff tear is suspected. Sudden, marked weakness following a specific injury, particularly difficulty lifting or holding the arm out, is a different situation from gradual-onset pain. Here, MRI for rotator cuff tear confirmation helps determine tear size and quality of the tendon, which influences whether surgical repair is worth considering.
Persistent instability after a dislocation. If the shoulder still feels as though it may slip out, imaging can identify labral or bony damage that affects management.
When symptoms are not improving despite appropriate treatment. If you have had a fair course of well-delivered physiotherapy and progress has stalled, imaging can help explain why and redirect the plan.
When surgery is being seriously considered. A surgeon needs anatomical detail to plan the procedure and counsel you about likely outcomes.
Red flags that need urgent medical assessment, not a routine scan
The Evidence-Based Interventions guidance lists specific features that warrant urgent referral rather than a routine outpatient pathway:
- Any history or suspicion of cancer
- A lump, mass or swelling around the shoulder
- Signs of infection, such as red skin, fever, or feeling generally unwell
- Trauma with pain and weakness
- Trauma, an epileptic fit or an electric shock followed by loss of rotation or an abnormal shoulder shape

When Is an MRI Usually Not Needed?
For several common presentations, treatment can reasonably begin after a clinical assessment, with imaging held in reserve:
- Gradual-onset shoulder pain with no specific injury
- Early frozen shoulder (adhesive capsulitis), which is usually identified through a characteristic pattern of restricted movement
- Mild rotator cuff-related shoulder pain or subacromial pain
- Postural or work-related shoulder pain
- Pain that came on after a change in training load or an unusually heavy gym session

- Has a proper physical examination been done first?
- What specific question is this scan meant to answer?
- Will the result actually change my treatment?
- Have I had a fair trial of appropriate rehabilitation?

Can Physiotherapists Diagnose Shoulder Problems Without MRI?
Yes. Many common shoulder problems can be assessed clinically without an MRI initially. A physiotherapist can evaluate your symptoms, shoulder movement, range of motion, strength and functional limitations to develop a working diagnosis. If the findings are unclear, symptoms do not improve as expected, or a significant injury is suspected, further medical assessment or imaging may be recommended.
How Physiotherapists Assess Shoulder Pain Without an MRI?
A structured clinical assessment covers a lot of ground.
History. How the pain started, what aggravates and eases it, how it affects sleep, work and daily tasks, and whether there was a specific injury. This helps narrow the likely causes and determine whether further investigation is appropriate.
Movement assessment. Watching how the shoulder blade and arm move together, and where in the range symptoms appear, points towards particular patterns.
Range of motion. Comparing what you can move yourself against what moves when guided helps distinguish stiffness from pain-limited movement — the key difference between frozen shoulder and most rotator cuff problems.
Strength testing. Testing specific muscle groups identifies weakness and helps gauge whether it is pain-related or structural.
Clinical tests. Specific hands-on tests reproduce symptoms in patterns associated with particular conditions.
An important caveat: no single clinical test is definitive. Individual shoulder special tests have modest diagnostic accuracy when used in isolation, which is why they are interpreted together with your history, your movement pattern and your response to early treatment — not as standalone answers. Many common shoulder problems can be assessed clinically without an MRI initially, and where clinical findings do not add up, that itself is a reason to image.
